Reflux vs CMPA: Why They're Often Confused
If your baby cries after feeds, arches their back or struggles to settle, it's natural to wonder what's causing it.
One of the most common questions parents ask is:
"Is this reflux or could it be a cow's milk protein allergy (CMPA)?"
The two conditions can look surprisingly similar—but they're not the same, and understanding the difference can help families avoid unnecessary worry and treatment.

Why reflux and CMPA are often confused
Both conditions can cause babies to:
- Cry during or after feeds
- Be unsettled
- Bring milk back up
- Feed little and often
- Struggle to settle afterwards
Because the symptoms overlap, reflux is sometimes mistaken for an allergy.
What does CMPA usually look like?
According to paediatric gastroenterologist Dr Roberto Albani, true cow's milk protein allergy often appears very early in life and usually includes symptoms beyond reflux.
These may include:
- Repeated vomiting
- Skin rashes or hives
- Persistent diarrhoea
- Blood or mucus in stools
- Poor weight gain
Every baby is different, so parents should always seek medical advice if they're concerned.
Why changing formula sometimes seems to "fix" reflux
One reason CMPA is commonly suspected is because babies often improve after changing formula.
But that doesn't necessarily confirm an allergy.
Some specialist formulas are simply easier to digest and empty from the stomach more easily. That can reduce reflux symptoms without meaning the original formula caused an allergic reaction.
As Dr Albani explains:
"Feeling better on a new formula tells you it was gentler on the gut. It doesn't tell you the previous one caused an allergy."
Why getting the diagnosis right matters
The difference affects the advice families receive.
A diagnosis of CMPA may lead to:
- Maternal dietary changes while breastfeeding
- Long-term specialist formula
- Ongoing allergy management
Whereas uncomplicated reflux often benefits from:
- Feeding adjustments
